The Vigil Honor Petition and Founder’s Award Petition have been updated to include these new email addresses. These petition forms can be found on the OA’s website here. If you are using previous versions of the forms with individual’s email addresses, please discard those and use these updated forms.
As we enter the busy time for awards submission, it’s crucial that you use the current version of the forms available to ensure we can efficiently process your request. Remember, petitions should be received no less than 45 days before the induction or presentation to allow sufficient time for processing and shipping.
