Printable Forms
The Order of the Arrow has several forms used
in carrying out the OA program. Some of these forms
(such as the Vigil Nomination Petition form, the Founder's
Award Petition form and others)
are only available as part of the Lodge Program
Support Pak, which is
sent to lodges with their charter each year.

The national Order of the Arrow Innovation Award seeks to recognize chapters and lodges for their efforts in improving our program and provides a means to chronicle the best ideas, promoting and sharing them nationwide as "best practices." It recognizes innovation at every level of the organization, and as such, any idea that has been proven to positively impact the chapter, lodge, district, council, or Scouting will be considered.
Unit of Excellence Award Information

The national

Online Version Format
The online forms are in "Portable Document Format" (PDF).
