The Order of the Arrow has several forms used in carrying out the OA program. Some of these forms (such as the Vigil Nomination Petition form, the Founder's Award Petition form and others) are only available as part of the Lodge Program Support Pak, which is sent to lodges with their charter each year. National BSA forms

The national BSA web site has several official forms available on its forms page. These include the various BSA Medical Exam forms, Tour Permits, and more.

Note, this national BSA forms page is not part of the National OA web site, and is not maintained by the OA web site staff.

Online Version Format

The online forms are in "Portable Document Format" (PDF). Some forms include Fields in the PDF document, which allow users to fill out most items on the downloaded form, then print a completed version.
